Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.
The practice worked with the local mosques to encourage people to attend for cancer screening and with their children for childhood immunisations.
The practice support “Breathe Easy, Get Together” which is a wellbeing and support group for people living with chronic obstructive pulmonary disease (COPD).
The practice support “Bollyfit” which is a women’s only fitness environment which incorporates cultural music and dance to tackle obesity, isolation and depression and is aimed at South Asian, Black and other ethnic minority groups to improve the wellbeing and physical health of the attendees.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service did not meet national targets for the uptake of childhood immunisations. Similar to other local practices, the service performed lower than the England average in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer.
To improve uptake, the practice proactively contacted patients via telephone and text message. Staff also utilised incoming patient contacts as an opportunity to offer flexible, convenient appointment times.
