Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support. Clinical audits were used to identify patients at risk of developing long-term health conditions, enabling timely intervention and support. The practice promoted national health initiatives, including programmes aimed at reducing obesity and supporting smoking cessation. Additional support was available for carers, recognising the impact that caring responsibilities can have on their own health and wellbeing. Health information and educational materials was displayed in the reception area to increase patient awareness of available services and wellbeing resources. The practice made use of a social prescribing referral system, enabling patients to access a wide range of community-based services and support networks to improve their health and wellbeing.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

At the time of our assessment, the latest published information from the UK Health Security Agency (UKHSA) in June 2024 showed that the practice did not meet the World Health Organisation expected target of 95% for the number of children immunised against various infectious childhood diseases but they did meet the minimum 90% target. The latest published information from NHS Digital showed the practice’s uptake for cervical cancer screening did not meet the 80% national target. For people aged 25 to 49, it was observed at 72.9% and for people aged 50 to 64 years old, it was observed at 77.7%. The provider assured us they were engaging with non-attenders. For example, they went to local mosques and community centres as part of their community outreach to educate people about the importance of immunisations and cervical cancer screening.
