Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. The service referred some patients to social prescribers to help with social aspects of their health. We saw case studies of positive outcomes for patients.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service was slightly below the 90% minimum target for some childhood immunisations. The service explained they now telephoned the parents of all children who had not attended for vaccinations to explain the benefits and discuss future appointments. The latest published data showed that 70% of 25 to 49 year olds and 71.6% of 50 to 64 year olds had been adequately screened for cervical cancer, against a target of 80%. As well as contacting patients to encourage uptake the service promoted local initiatives to try to improve the uptake. These included an enhanced access service where patients could attend at a time convenient for them, and a drop-in mobile service in the area. The service performed better than the England average in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer.

The service had a programme for the carrying out of clinical audits, and these were repeated regularly.
