Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support. For example, the service supported a local homeless project providing vaccinations, and they supported a drug and alcohol service by providing wound care. The service hosted group sessions focusing on women’s health in relation to menopause and there was a therapy garden at the service which was open to the local community as a well-being space. Health promotion materials were available in waiting areas and on the service’s website.
Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including smoking cessation and tackling obesity.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service met the World Health Organisation (WHO) target for 4 out of 5 childhood immunisation indicators, achieving between 95% and 97%. The service was below the national target of 90% for children aged 5 who have received immunisation for Measles, Mumps and Rubella (two doses of MMR) with a score of 88.5%. The service had a system for increasing uptake of childhood immunisations via phone calls and emails.
NHS England data (June 2024) showed the service achieved 81% uptake for cervical screening for women aged 25-49, and 79% uptake for women aged 50-64. This was in line with the national target of 80%. The service offered screening clinics from 7:30am weekdays, 4 days a week and additional clinics on a Saturday. They also undertook cervical screening for people within the wider community.
The service conducted clinical audits as a system to improve outcomes. For example, an audit was conducted to monitor antimicrobial prescribing in December 2024. The audit identified an opportunity to improve the prescribing of 5-day antibiotic courses, rather than 7-day courses, in line with best practice guidance. The service acted by reviewing prescribing guidelines. A follow-up audit in April 2025 showed improvements, with the proportion of antibiotic prescriptions issued for 5 days increasing from 63.9% to 88.8% which was above the 75% target set by the local Integrated Care Board (ICB).
