During our last assessment in April 2025, we found that the practice did not have a safeguarding register for adults and some adults did not have the appropriate codes. We had also found that some staff did not have the required learning of safeguarding for their job role. During this assessment, we found no concerns regarding this.

The service worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. They concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The service shared concerns quickly and appropriately. Safeguarding policies were in place and known to staff, who were appropriately trained in safeguarding procedures. The practice maintained a list of vulnerable people and acted on concerns working in partnership with other organisations.

Children on the safeguarding register were coded appropriately and had households that were also coded. We looked at 8 adults who were coded appropriately. Out of the 8 adults we looked at, 4 did not have households coded. The practice did not have a policy to code household members of adults on the safeguarding register. The 2 children we looked at did have the households coded. The practice recognised the complexity of coding in regard to household members for adults and were going to hold a clinical safeguarding meeting to review the most appropriate method for coding households of adult safeguarding patients.

The service made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. We saw examples of staff requiring supervision have a debrief at the end of each day that they worked.

There were a range of clinical and non-clinical roles within the practice. We found most training was up to date, learning needs and development of staff was managed appropriately, and staff were working within their agreed areas of competence. Safe recruitment practices were followed.

During our last assessment in April 2025, we found gaps in training and recruitment files for some salaried and locum staff. During this assessment, we found that most staff had all training and all staff had their recruitment files. We found 1 locum staff to be missing Mental Capacity Act training, Chaperone training and Learning disability Autism training. This was actioned after the site visit, and all members of staff were up to date with their training.
