Effective
We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this. At our last inspection, we rated this key question as Good. At this inspection, the rating remains the same.

Supporting people to live healthier lives
The practice supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. During our inspection, we saw how the practice had identified and acted upon an increase in the prevalence of diabetes in the population. Staff supported national and local priorities and initiatives to improve population health, including stopping smoking and tackling obesity.
The practice utilised their website and social media page to promote health promotion advice and support health campaigns. This included sharing health promotion advice and updates about recent outbreaks of disease in a neighbouring county.
To further support health promotion, the practice actively contributed to safe and effective research. The practice was a registered research ready practice and identified patients who may be eligible for different clinical trials. Recent studies where the practice had identified patients to take part included an atrial fibrillation (an irregular heart rhythm) research initiative.
Monitoring and improving outcomes
The practice routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The practice met national targets for all the uptake of childhood immunisations. The practice performed in line with England averages in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer.
We saw a programme of clinical audit had been undertaken over the last 12 months, this included a variety of 2 cycle audits including a 2-cycle lipid/statin management audit. Audit activity was prompted following significant events, in alignment to local and national health priorities and in areas the GPs had specialist interests in.
