Our view of the service
Date of Assessment: 12 February 2026. The Glebe Family Practice is a GP practice and delivers service to approximately 4,825 patients under a contract held with NHS England. The National General Practice Profiles states that that the ethnic make-up of the practice area is 77% White, 10% Asian, 7.5% Black, 3.5% Mixed and 2% Other. Information published by Office for Health Improvement and Disparities shows that deprivation in the practice population group is in the 4th decile (4 of 10). The lower the decile, the more deprived the practice population is relative to others.
The service had made improvements and is no longer in breach of regulations. The practice had clear responsibilities and roles. Governance structures had improved and they now had established and embedded systems of oversight for monitoring vaccines and task management.
