We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

Staff supported people to live healthier lives and monitored their care and treatment to ensure they received positive and consistent outcomes. Staff regularly reviewed people’s care and worked with other services to achieve this.

Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, which included for example, people in the last 12 months of their lives and those with a long-term condition. The service had effective arrangements in place to provide a range of health checks, including NHS Health Checks for eligible people, physical health checks for people with a severe mental illness, and annual health checks for people with a learning disability. In the past 12 months, 100% of people with a severe mental illness had received a health check. All eligible people with a learning disability were offered a health check, and 100% of those who wanted one received it.

Service leaders and staff supported national priorities and initiatives which included for example, stopping smoking and tackling obesity. A range of information was available in the waiting room area and on the service’s website which signposted people to information and local and national support groups to encourage healthier living. Staff we spoke with gave examples of events, for example influenza vaccination campaigns at the service, to raise awareness and encourage uptake. Service leaders had worked with the Patient Participation Group to update the service’s social media page regularly with health promotion information. This information was also shared in the local parish magazine.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met national targets for the uptake of childhood immunisations. The service performed above the national average for breast and bowel cancer screening. Cervical screening uptake was slightly below the national target. Leaders were aware and had taken action to increase nurse capacity. Staff told us the uptake of cervical screening had increased, and the practice provided unverified data which supported this view. Effective recall systems were in place and flexible appointment times, which included early mornings.
