During this inspection we found that people’s ability to make positive choices about their health and have control over their own care was reduced by the ineffective systems and processes the provider proposed to manage patients with long-term conditions. If patients cannot access timely reviews of their condition, it can reduce their ability to manage their condition effectively and minimise the impact of that condition on their lives. Patients who do not have regular, comprehensive medicine reviews may experience side effects or suboptimal symptom control for longer than is necessary were they supported to make informed choices about their treatment. Given that the provider had not supplied clear and unequivocal evidence to support the effective and timely management of these patient groups, it was not clear that patients would have independence, choice and control.
