Leaders and managers supported staff, and all staff we spoke with were clear on their individual roles and responsibilities. Managers met with staff regularly to complete appraisals. Managers held regular practice meetings with staff, during which they discussed clinical concerns and emerging risks and this was recorded clearly.

At the last assessment in July 2025, we found gaps in training and recruitment files for staff. the practice did not have a fire risk assessment and the health and safety risk assessment was overdue. The practice did not have records to confirm fire alarm tests were carried out. At this assessment, the practice has had calibration, a fire risk assessment and a health and safety assessment all carried out. We found that all staff had the required training and recruitment files in place.

During our clinical search, we found 11 patients with potential missed diagnosis of diabetes who should be on diabetic register with one patient dating back to 2024. All patients we looked at were on a pre-diabetic register which ensured regular and active recall. However, the patients would not have had all the services offered if they were listed on a diabetic register, such as being offered Diabetic Eye Screening. Some patients we looked at were known to be diabetic by the practice but were not coded as such. The practice have acknowledged these points and will be adding a formal diabetes code once confirmatory criteria are met, even where the patient still needs direct discussion with the practice. Also, the practice will be introducing an escalation search for patients with diabetes, retinopathy/high HbA1c (a measure of the average blood glucose over a two to three month period) and no active glucose-lowering medication.

During our clinical search, we found that the documentation, in regard to asthma exacerbation, was not in line with asthma guidelines by the practice. Following the assessment, the practice said it would introduce a rescue pack follow-up process, which will be documented using a standardised template in the patient clinical record to make safety-netting and follow-up visible.
