Supporting people to live healthier lives
The service supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff encouraged people to live healthier lives and, where possible, reduce their future needs for care and support.
Staff focused on identifying risks to people's health, including those approaching the last 12 months of their lives and those with caring responsibilities. The service supported national priorities and initiatives to improve population health, including smoking cessation programmes and increasing the uptake of learning disability health checks.
The service demonstrated its commitment to supporting carers through dedicated carers' weeks and the appointment of a named lead responsible for engaging with carers and promoting access to carers' health checks. The service also produced quarterly newsletters and was supported by an active Patient Participation Group (PPG), which contributed positively to service development and patient engagement.
The provider demonstrated that feedback and areas for improvement discussed within regular meetings were acted upon. This included strengthening clinical representation and ensuring consistent attendance and involvement from members of the clinical team to support service development and improve outcomes for people using the service.
Monitoring and improving outcomes
The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The service performed well and met national targets for the uptake of childhood immunisations. The service performed slightly below the England average in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer.
