Effective
We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.
This service scored 75 (out of 100) for this area. Find out what we look at when we assess this area and How we calculate these scores.

Supporting people to live healthier lives
The practice supported people to manage their health and wellbeing to maximise their independence, choice and control. The practice supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Digital flags were added to clinical records of individuals who had been identified as having specific needs or preferences. This enabled staff to consider how they provided personalised care with respect to wellbeing and choice.
Health checks were offered to specific groups including carers and those with learning disabilities, which ensured suitable support was in place. Nominated staff were responsible for co-ordinating reviews and support services for these groups and social prescribing was also utilised where appropriate.
Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity, and we observed information about national health campaigns and local support groups displayed at the practice. Participation in national cancer screening programmes was monitored by the practice and work was undertaken to improve participation.
Monitoring and improving outcomes
The practice routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The practice was aware that they had not met national targets for the uptake of childhood immunisations and had carried out quality improvement work to address this. They had worked to improve trust with minority groups and those hesitant to or refusing immunisations, and had seen improvements in every age specific indicator.
Work has also been undertaken to improve the uptake of cervical cytology screening in the practice population. Staff identified a better response to telephone calls as opposed to text messages when inviting people to book for screening, and this approach increased uptake overall.
In February 2026 the practice had 440 responses to the NHS Friends and Family test, and 98% of responses were positive. The practice provided information about how they have analysed negative responses to this and other surveys, to ensure improvements were made. For example, in response to a theme of clinicians running late for appointments, systems for providing information to patients had been improved.
Consent to care and treatment
