Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.People’s needs were assessed and reviewed within 48 hours, in line with the provider’s policy, to ensure staff had a clear understanding of how to support them. Care plans reflected people’s involvement and provided guidance on delivering care in line with their preferences. People told us they were familiar with their care plans and felt involved in decisions about their care. Relatives confirmed they were able to contribute and could speak with managers if needed.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Most staff said they worked well together and shared information to make sure people got the right care. Records showed health professionals were involved when needed, and their input was clearly reflected in people’s care plans.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. People told us staff were well trained and knew what they were doing, which supported a good quality of life. Monitoring tools were used to track health and care needs, so action could be taken when needed. Care records showed people’s support was reviewed regularly to make sure it stayed up to date.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.Care plans outlined people’s capacity and their ability to consent to decisions. Where required, mental capacity assessments had been completed. While meetings took place to discuss what was in the person's best interest when necessary, documentation did not consistently reflect the full decision-making process such as the range of options considered, the rationale for rejecting alternatives and the reasons behind the final decisions. Despite this, there were no concerns regarding the quality of care or treatment provided. Decisions were made collaboratively with the involvement of relevant people, ensuring people’s best interests remained central to the process.
