Person-centred Care

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs. As evidenced throughout this report, providing person-centred care was at the heart of service delivery.

In the other areas we looked at in this quality statement, we saw the provider was transitioning to a new digital care planning system. Staff and leaders had been working hard to ensure the new system met the unique needs of the service, taking learning from the providers other locations where the system was now live. Most staff were supportive of the transition, recognising the need to move from multiple paper-based to digital records.

Care provision, Integration and continuity

The provider understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. As evidence throughout this report, people were supported to access health and social care professionals when needed. Any advice or guidance was recorded in people’s care plans and was understood and followed by staff.

Providing Information

The provider supplied appropriate, accurate and up-to-date information in formats that were tailored to individual needs. The accessible communication needs of people were also fully considered and acted upon through the providers full and comprehensive trusted assessor assessments. This ensured people had an information plan, and the service could access the necessary support equipment.

Listening to and involving people

The provider made it easy for people to share feedback and ideas, or raise complaints about their care, treatment and support. Staff involved people in decisions about their care and told them what had changed as a result. Comments from people included, “Communication is very good we have no complaints whatsoever” and “Killelea communicate well with me, I’m aware of what is going on by everyone who was supporting me.”

Equity in access

The provider made sure that people could access the care, support and treatment they needed when they needed it. Staff understood the importance of ensuring people had access to the right health and social care services. People were supported to access treatment when needed, and families were informed and involved when appropriate.

Equity in experiences and outcomes

Staff and leaders actively listened to information about people who were most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. As evidenced throughout this report, staff understood and how to reduce health inequalities that affected outcomes for people. Staff and leaders ensured peoples’ social and healthcare needs were fully considered and met.

Planning for the future

Score: 3

People were supported to plan for important life changes, so they could have enough time to make informed decisions about their future. The service is a rehabilitation service and does not routinely provide end‑of‑life care. However staff recognised that people’s needs may change. Where a person becomes imminently end of life, the service responds appropriately, working with external professionals to ensure the person is supported with dignity and respect in accordance with their wishes. Planning for the future is embedded within holistic assessment and reviewed as needs change.
