Well-led
Well-led – this means we looked for evidence that service leadership, management and governance assured high-quality, person-centred care; supported learning and innovation; and promoted an open, fair culture. At our last assessment we rated this key question as good. At this assessment the rating has remained as good. This meant the service was consistently managed and well-led. Leaders and the culture they created promoted high-quality, person-centred care.

The provider had a shared vision, strategy and culture. This was based on transparency, equity, equality and human rights, diversity and inclusion, engagement, and understanding challenges and the needs of people and their communities.

Governance, management and sustainability
The provider did not always have clear responsibilities, roles, systems of accountability or good governance. They did not always act on the best information about risk, performance and outcomes, or share this securely with others when appropriate.
The providers checks did not consistently identify pressure cushions were in poor condition. Wheelchair maintenance checks did not include heel straps and had therefore failed to identify missing or broken straps increasing the risk to people. Although the provider safely recruited staff potential information of concern was not always passed to the registered manager compromising the effective supervision of staff. The providers review of people’s care and support plans had not consistently identified where peoples decisions and choices were not clearly recorded to ensure their rights were preserved
Although leaders took immediate action to update these processes during the inspection, this demonstrated that governance systems were not sufficiently effective prior to inspection. In addition, while monthly maintenance audits were reported to be completed, these were not consistently recorded or documented, limiting oversight and accountability.
The providers review of people’s care and support plans had not consistently identified where people’s decisions and choices were not clearly recorded to ensure their rights were preserved.

Learning, improvement and innovation
The provider focused on continuous learning, innovation and improvement across the organisation and local system. They encouraged creative ways of delivering equality of experience, outcome and quality of life for people. They actively contributed to safe, effective practice and research.
