Safe environments

Score: 3

The service detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care.

The service had contracts to ensure the premises was maintained. The service operated out of a rented building. Staff and leaders completed health and safety risk assessments and undertook audits to ensure they had identified and addressed all risks. Through these audits the service had identified some hand-washing sinks that were not compliant with latest up to date guidance. The sinks were added to the health and safety risk register.

Through audit of the building and whilst checking to make sure the building was safe, the provider had identified a potential risk of subsidence which manifested itself as gaps in the walls and in some of the ceilings. These risks had also been put onto the risk register.

The service had a business continuity plan which was regularly reviewed and outlined how the service should continue to operate in the event of a disruption.

Safe and effective staffing

Score: 3

The service made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

The service employed a range of clinical and non-clinical roles, which included GPs, nurses and pharmacists. Mossley group practice is supported by a not-for-profit organisation GoToDoc Limited. GoToDoc Limited determine which training should be completed by the staff employed to work at the practice. Based on best practice not all staff had received all relevant training. For example some staff had completed chaperone training but not all. None of the staff employed had complete mental capacity act or sepsis awareness training. Leaders ensured staff were up to date with other training which the service had deemed mandatory and operated within their agreed areas of competence.

The service followed safe recruitment procedures when employing staff, which were in line with national legislation. This included identity checks, review of qualifications, obtaining of professional references and a criminal records check.

Infection prevention and control

Score: 3

The service assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

The practice had a designated infection, prevention and control lead and all staff had had relevant training. Cleaning schedules were in place and followed. Risk assessments and audits, including handwashing audits were completed. Some of the sinks in use were not compliant with the most recent standards and had overflow holes within them. Overflow holes can be difficult to clean effectively, presenting a potential risk of bacterial growth and cross-infection. Risk assessments had been completed and the service was aware of the need to change these.

The clinical rooms were of a good standard. Handwashing facilities with good levels of personal protective equipment (PPE), hand towels and liquid soaps were available for use. The privacy curtains we checked were all clean and in good state of repair.

Appropriate levels of liquid soap and hand towels were available in the public and staff toilet facilities.

We witnessed clinical staff were bare below the elbow whilst on duty which promoted effective hand hygiene.
