The extended hours service is commissioned by all 7 Wakefield Primary Care Networks and covers a population of around 401,000 people. The provider also delivers a number of separately contracted projects and services which includes urgent care, respiratory care projects and health checks and support for specific groups. In addition, the service also operates pop-up sites across the locality to support the primary care system and as part of project work. As part of our assessment, we visited the main site at Trinity Medical Centre, and the service delivered from Ossett Surgery.

Operating days and times vary across the locations operated by the provider, but include late weekday, weekend and bank holiday appointments. Appointments include face to face consultations and remote consultations. Outside of the operating times for the service patients can access support from NHS111 Local Care Direct Limited.

Services are provided by a mix of substantive and sessional clinical and non-clinical staff.

This assessment considered the demographics of the people using the service, the context the service was working within and how this impacted service delivery. Where relevant, further commentary is provided in the quality statements section of this report.

Conexus Healthcare CIC also delivers a number of separate services including a clinical assessment service and several other projects, such as lung health checks, a specific respiratory project for ex-mining communities and workplace health checks. These are subject to a further stand-alone assessment report. This assessment report focuses on the extended hours service, a main respiratory project, and a developing women’s health service.
