Our view of the service
We carried out this announced focused on-site inspection on 9 January 2026.
The inspection was led by a CQC inspector who was supported by a second CQC inspector.
We found the practice had not met all regulations.
There was ineffective leadership and a lack of oversight of the governance systems to sustain continuous improvement. Previous inspections had highlighted issues which had been responded to, but these improvements had not been embedded and maintained.
Recruitment and training policies reflected current legislation, but these were not followed.
The practice did not have effective oversight to identify and manage risks, including control of hazardous substances, and inconsistencies in infection prevention and control processes.
The practice had appropriate safeguarding and medical emergency processes. However, not all staff were aware of these, and evidence of training was not consistently obtained.
Staff provided care and treatment in line with current guidance. They treated patients with dignity and respect and ensured access to care, support, and treatment when required.
Mr Osman Mohammed is in Bradford and provides NHS and a small amount of private dental care and treatment for adults and children. The practice also offers evening urgent care services via the NHS 111 service.
The practice is not accessible to people who use wheelchairs. On street parking is available near the practice.
At the time of our inspection, there was a total of 6 staff. The dental team includes 2 dentists, 2 dental nurses and 2 receptionists. The practice has 2 treatment rooms, but only 1 is in use.
During the inspection visit we spoke with the principal dentist, both dental nurses and 1 receptionist. We looked at practice policies, procedures and other records to assess how the service is managed.
The provider was not complying with Regulation 17 Good Governance. Full details of the regulation the provider was not meeting are within the inspection report.
In instances where CQC has begun a process of regulatory action, we may publish this information on our website after any representations and/or appeals have been concluded, if the action has been taken forward.
