The provider had established effective systems and processes to assess, monitor and improve the quality and safety of the service. Their improved systems enabled them to identify where quality and safety were being compromised and to respond appropriately and without delay. The practice had contracted an external specialist risk assessment company to carry out updated risk assessments to support the manager.

There were now systems to ensure policies and procedures were followed. The safeguarding policy had been improved and additional meeting and training held to ensure these were embedded.

Processes were in place to identify and manage risks in relation to health and safety, fire safety, recruitment, ensuring the servicing of equipment at appropriate intervals and the oversight of infection prevention and control processes.

The provider had updated the incident policy and implemented systems for reviewing and investigating when things went wrong. In particular, processes to document and investigate incidents, accidents and significant events. We highlighted a retrospective review of the previously documented incidents should be carried out to identify any learning or necessary actions.

The practice had also made further improvements: Clinical audits had been improved, these now included conclusions and action plans.
