Effective – this means we looked for evidence that people’s care, treatment, and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

At our last assessment we rated this key question good. At this assessment, the rating has changed to requires improvement. This meant the effectiveness of people’s care, treatment and support did not always achieve good outcomes or was inconsistent. The service was in breach of legal regulations in relation to how people were not supported in a person-centred way and protecting people from avoidable harm.

Assessing needs

The provider did not make sure people’s care and treatment was effective because they did not check people’s care records contained accurate and personalised information. Information had been copied from historic assessments and care records and pasted into recent care plans without consideration to its accuracy or relevance. Information relied upon was not consistently recorded across people’s care records and there were discrepancies relating to personal information, risks, and needs. People’s voices, personal preferences and wishes were not always reflected or explored within their care plans.

Monitoring and improving outcomes

The provider did not robustly monitor people’s care and treatment to continuously improve it. Reviews of care plans and assessments could not be relied upon as being accurate. Care plans did not reflect lessons learnt, people’s aspirations or outcomes. Positive behaviour support (PBS) plans were not updated following incidents to ensure support strategies were appropriate, consistent, positive, or respectful. There were discrepancies between incidents reports and people’s daily notes which meant information could not be relied upon to shape people’s care experiences and outcomes.

The provider had not always told people about their rights around consent. CCTV was used to monitor the exterior and garden of one supported living scheme without tenants’ knowledge of consent. Following our feedback the provider has informed us CCTV is no longer used. Applications were made to the Court of Protection to legally authorise people’s deprivation of liberty. These included 1-1 staffing and locking the kitchen door within a supported living scheme.
