At our last inspection we found patients’ healthcare applications were triaged by administrators with no clinical guidance or oversight, and this was not known to senior managers. This created a risk that patients might not be seen by the appropriate clinician and a risk of urgent need not being identified and addressed in a timely manner.

At this assessment we found that the processes had been revised. All healthcare applications are triaged by either a GP, an ACP or a senior nurse to ensure that individuals are directed to the most appropriate healthcare professional, based on the urgency and complexity of their healthcare needs, and that appointments are allocated safely and effectively. This helps to ensure timely access to care and supports consistency in clinical decision-making.

At our last inspection we found that not all patients with long term conditions had a care plan. The audit in place to monitor care plan completion rates did not include all long-term conditions.

At this inspection we found that in the cases we reviewed a care plan was in place or the patient had a booked appointment to complete these care plans. We found that most of these care plans were personalised to the patient, but that some lacked detail to help ensure that all staff were aware of how to fully support the patient, for example how a person might present before, during and after a seizure. We also found that some areas on the care plan templates where not appropriate for the prison environment, such as making reference to mobile phones and contacting 999. The provider told us that this was to ensure care plans were relevant for when patients left the prison.
