Our view of the service
Date of Assessment 6 November to 3 December 2025. Woodfield Care Home is a care home providing personal and nursing care for up to 36 older people, some of whom may be living with dementia, have a sensory impairment or a physical disability. During the assessment 31 people were living at the service. The local authority had placed a suspension on new admissions to the service from 3 October 2025. This was due to concerns relating to governance, oversight and lack of clinical lead and the suspension continued throughout our assessment. This assessment was carried out due to concerns relating to poor documentation, poor staff culture, staff shortages and ineffective communication with families we had received about the service.
We found 3 breaches of the legal regulations in relation to safe care and treatment, staffing and governance.
We observed a senior staff member providing significant support and guidance to staff when moving and handling people. We found this support was not embedded; in the senior staff member’s absence; we observed unsafe moving and handling practice carried out by staff. Medicines were not managed safely. We identified concerns relating to administration records, following ‘As and when required’ medicine protocols, stock balances, safe storage and record keeping. Staff did not always assess or mitigate risks to people's health and safety. People did not always have care plans to guide safe practice. People, who chose to stay in their rooms, did not have access to call bells and staff did not respond to their needs in a timely way. Governance systems and audits were not effective in identifying or addressing areas for improvement despite the local authority working closely with the provider to provide support, training and monitoring of the service for a considerable time.
However, people living at the service told us they felt safe and well care for and liked living at the service. We found the service to be clean and odour free, improvements to the decoration were ongoing during the assessment.
CQC have decided to take civil enforcement action against the provider, we will publish this information on our website after any representations have been concluded.
People's experience of this service
People told us they liked living at the service and felt safe and looked after. Relatives provided mixed feedback. Some relatives reported their loved ones were well looked after and safe, however some relatives felt their loved ones were not safe and raised concerns around poor care and communication from the provider. People told us staff were very friendly, always kind and caring and treated them with dignity and respect. One person told us, “It is wrong to call them staff, they are family.” Some people shared concerns about staffing levels and said staffing could be improved. People told us the food was nice and noted improvements since new staff had started in the kitchen. Some relatives felt communication was good and said they were kept informed about changes and care. Some relatives had been informed the provider was not admitting new people but did not know the reason for the decision. Due to people lacking capacity and not able directly to tell us about their experience, we used a structured observation tool to assess whether they received good care. We found people spent time in the lounge with staff; however, interactions were task based rather than meaningful.
