Our view of the service
Thornhill Nursing Home is a service providing nursing and personal care for up to 42 people, some of whom are living with dementia. At the time of our assessment there were 38 people living at the service.
The assessment was carried out to follow up on enforcement action issued following breaches of regulation at the last assessment. It commenced on 6 January 2026 and was completed on 19 January 2026. Site visits took place on 6 January 2026 and 13 January 2026 which included an out of hours visit. The provider was previously in breach of regulations relating to safe care and treatment; safeguarding people from avoidable harm or abuse; safe staffing; dignity and respect; person centred care and good governance. The provider had made improvements and was no longer in breach of these regulations. Oversight of the service had improved. However, further embedding was needed to ensure areas of concern were fully addressed and stable leadership maintained. Aspects of risk management, environmental security and medicines management also needed further improvements to ensure all people received safe care and treatment. Aspects of documentation and record keeping also needed to be addressed. The culture at the service had improved, the manager and staff were open and positive. They had developed good relationships with people and relatives which enhanced people’s experience. There was an overall commitment to addressing any outstanding concerns and developing the service.
This service has been in Special Measures since 13 March 2025. The provider demonstrated improvements have been made. The service is no longer rated as inadequate overall or in any of the key questions. Therefore, this service is no longer in Special Measures.
People's experience of this service
People were supported by kind, caring staff who promoted their independence and respected their privacy and dignity. People and relatives consistently gave positive feedback about staff and how they were cared for. Comments included, “[The deputy manager] is excellent. He was here Christmas day dressed as Santa and gave everyone a hug,” “It’s comforting in a difficult situation for family that all the staff are so good and so nice” and “Every single one of the staff are lovely.” People had their personal care needs met and had continued access to food and fluids, which were appropriately monitored when needed. Further improvements were needed to ensure people and relatives were consistently involved in care planning and reviews. People’s records also required timely updating following incidents of a safeguarding nature to ensure interventions were in place to protect them from harm. Some activities were timetabled and were taking place. Some people were meaningfully engaged however, further progress was needed to ensure all people could participate in something they enjoyed, and access outdoor space. Overall people and relatives gave positive feedback about their experience of the service, commenting on improvements that had been made. Feedback included, “From the minute I walked in I thought this is amazing. [Name] is always clean and their bedding clean. It is changed every day. The food is amazing” and “I’ve got the best bed here. I tell staff if I have any problems and they always sort it out. I'm not really bothered about food and eating but they always come in and offer me a choice. I asked the other day for a banana butty because I fancied it. They went and got me one. You can't ask for more than that.”
