At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People’s needs were assessed prior to them using the service. However, 1 relative commented that they felt the assessment process was not very personal. They commented, “The assessor wasn’t really interested in getting to know [family member] as a person.” We raised this feedback with the manager for them to review. Other relatives did not raise any concerns about the assessment process.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

Staff used evidence-based tools to assess risks to people. For example, national tools for assessing risks for malnutrition and pressure ulcers

How staff, teams and services work together

Score: 3

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. Staff told us they worked well together. Professionals said the service worked effectively with them. One professional commented, “I confirm that we have been working with Aims Homecare for a while now and I found them to be resourceful, efficient, and reliable.”

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

People had information about their health conditions in their care plans and how these conditions affected them. Staff had received training in specific health conditions, such as epilepsy, to support people effectively.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

People and relatives said staff supported them to achieve positive outcomes. One relative commented, “The carers check that [person] is eating and drinking properly, particularly during this very hot spell.”

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

Staff had received training in the Mental Capacity Act. People’s capacity had been assessed and best interest decisions had been recorded for people.
