At our last assessment we rated this key question good. At this assessment the rating has remained good.

Assessing needs
The provider had systems and processes in place to assess and review people’s health, care, wellbeing and communication needs with them. Leaders were passionate about their service. They were open to providing support to anyone they felt the service could safely support, or learn to support, no matter how complex their support needs were. The provider kept the service’s capacity under review to ensure when they had new packages of care it did not compromise the care of people they already supported. The provider had different pre-assessment processes in place depending on the specialism of care they were providing to people. This meant they could tailor their approach to meet people’s specific needs. For example, for people being discharged from hospital into the community with rehabilitation support the provider had a specific team of care staff trained in rapid discharge who liaised with the person and their family to establish what support they wanted and needed. For people with complex care support needs the provider built a team specifically for the person with the person and their family.

Supporting people to live healthier lives
Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. The provider’s clinical team supported people and their families to monitor people’s health so they could promptly respond to any signs of deterioration. This early intervention supported the reduction of hospital admissions for people. The service made sure referrals to appropriate health services were made quickly when people’s needs change and acted swiftly on their recommendations. The provider worked with families and schools to support children’s skills development or advocating for people to ensure they received the support they needed form health and social care professionals.
