Our view of the service

The assessment took place between 18 March and 3 April 2026.

Nurse Plus Exeter is a domiciliary care agency providing personal care support to people in their own homes. At the time of our assessment 11 people were receiving personal care, some were older people and some were living with dementia. The service does not routinely look after people with nursing needs but is registered to do so as staff have supported people with complex needs in the past.

We visited the location's office on 18 March 2026. We gave the service notice of the inspection. This was because it is a small service and we needed to be sure the provider would be available to support the inspection.

At the time of the assessment there were no people using the service who had a learning disability. As a result, we were not able to assess the service against ‘Right support, right care, right culture’ guidance to make judgements about whether the provider guaranteed people with a learning disability and autistic people respect, equality, dignity, choices, independence and good access to local communities that most people take for granted.

People received safe,personalised care that met their needs. People were supported by a consistent team of key workers who knew them well, including how they liked to be supported and what support they required to manage any risks to their safety. Clear care records were in place outlining people’s support needs. The manager undertook regular review of people’s care and updated care plans as and when required.

Systems were in place to promote safe medicines management. Staff followed procedures to protect people from the risk and spread of infections. Staff provided any support required with accessing healthcare services, and staff liaised with relatives promptly if they had any concerns about a person’s health or welfare.

Staff were well supported in their role with good access to training and supervision. Staff felt able to speak openly with the registered manager. The registered manager had implemented systems to facilitate good communication, and processes to record and learn from any incidents, accidents or complaints that occurred. A regular programme of spot checks were in place to review the quality and safety of care delivery.

People's experience of this service

People and relatives were happy with the care they received. They felt safe receiving care and support from Nurse Plus Exeter. A relative told us, “I can’t fault them for their communication. They have taken such a burden off me. I am very pleased. They go above and beyond.” People received support from a consistent team of care workers who got to know people well. Relatives said care workers had developed good relationships with their family member and were kind and caring. Another relative told us, “I have two carers that come and they are both marvellous.” Care workers ensured people were listened to and took the time to get to know the people they were supporting, including finding out what was important to them and how they preferred to be supported. Care workers involved people in their care and respected their decisions. People felt able to raise any issues. Care workers encouraged people to remain as independent as possible, providing support only where necessary.
