Effective

Requires improvement

10 February 2026

Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

At our last assessment we rated this key question good. At this assessment the rating has changed to requires improvement. This meant the effectiveness of people’s care, treatment and support did not always achieve good outcomes or was inconsistent.

This service scored 58 (out of 100) for this area. Find out what we look at when we assess this area and How we calculate these scores.

Assessing needs

Score: 2

The provider did not always make sure people’s care and treatment were effective because they did not always check and discuss people’s health, care, wellbeing and communication needs with them when assessing needs. Assessment took place, but we saw that some contained gaps and lacked information on the support people wanted and needed.

Delivering evidence-based care and treatment

Score: 2

The provider did not consistently plan and deliver people’s care and treatment with them, including what was important and mattered to them. However, people received effective support with eating and drinking and spoke positively about meals at the service. One person said, “The food is nice, you get 2 choices (and) plenty of drinks and snacks.”

How staff, teams and services work together

Score: 3

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. We saw examples of how staff were working with healthcare partners to transition people out to more appropriate care settings.

Supporting people to live healthier lives

Score: 3

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and access healthcare partners where needed. One person told us, “If I need a doctor or someone they get them for me. I just tell them if I’m not well.”

Monitoring and improving outcomes

Score: 2

The provider did not always routinely monitor people’s care and treatment to continuously improve it. We saw gaps in people’s risk assessments and care plans, which meant outcomes could not be effectively monitored.

The provider did not always tell people about their rights around consent and did not always respect their rights when delivering care and treatment. For example, where decisions were made in people’s best interests it was not always clear who had participated in these to ensure that people’s voices were heard.
