The practice had some systems to identify and manage risks effectively, and staff described the processes. This included sepsis awareness and lone working.

Staff demonstrated an open culture in relation to people’s safety. They felt confident that risks were well managed at the practice. A sharps risk assessment was in place and staff were aware of the importance of reporting inoculation injuries.

Staff could access emergency equipment and medicines that were checked in line with national guidance. They knew how to respond to a medical emergency and had completed training in emergency resuscitation and basic life support every year. We found there was no child-sized self-inflating bag and additional sized needles were needed. They took immediate action to order the missing items. The practice should ensure staff are aware of medicines held in ampoules and are confident in the process to use these.

The premises were visibly clean, well maintained and free from clutter. Hazardous substances were risk assessed, clearly labelled and stored safely.

We saw satisfactory records of servicing and validation of equipment in line with manufacturer’s instructions.

The practice had arrangements to ensure the safety of the X-ray equipment and the required radiation protection information was available.

The practice should take action to appoint a competent person to carry out preventive and protective measures, taking into account The Regulatory Reform (Fire Safety) Order 2005 and ensure ongoing fire safety management is effective. A fire risk self-assessment had been completed. However, there had been no training undertaken to assure the competency of the risk assessor, we saw some areas of the premises had no fire detectors or extinguishers, and a rear fire door was locked. The provider gave assurance they would arrange for a competent person to review the arrangements for fire safety. We saw staff received fire safety awareness training, carried out and documented checks of fire safety equipment and participated in regular evacuation drills.

The practice had systems for appropriate and safe management of medicines. NHS prescription pads were kept securely, and a log was in place to monitor and track their use.

Safe and effective staffing

The practice had a recruitment policy and procedures that reflected relevant legislation, to help them employ suitable staff.

The practice ensured clinical staff were qualified, registered with the General Dental Council and had appropriate professional indemnity cover.

Newly appointed staff had an appropriate role specific structured induction.

Staff had the skills, knowledge and experience to carry out their roles. They told us that there were enough staff on duty at all times. They demonstrated knowledge of safeguarding and were aware of how safeguarding information could be accessed. Staff knew how to escalate safeguarding concerns within the practice and externally.

The practice ensured staff training, including continuing professional development, was up-to-date and reviewed at the required intervals.

There were effective processes to support and develop staff with additional roles and responsibilities. Staff discussed their learning needs, general wellbeing and aims for future professional development during annual appraisals, one-to-one meetings, during clinical supervision, practice team meetings and ongoing informal discussions.

Staff felt respected, supported and valued, and they were proud to work in the practice.

Infection prevention and control

The practice had infection control procedures that reflected published guidance.

Staff received appropriate training and demonstrated knowledge and awareness of infection prevention and control processes.

Staff used personal protective equipment and decontaminated dental instruments after use, in line with national guidance. We saw, and staff confirmed that single-use items were not reprocessed.

The practice should take action to implement any recommendations in the practice's Legionella risk assessment, taking into account the guidelines issued by the Department of Health in the Health Technical Memorandum 01-05: Decontamination in primary care dental practices, and having regard to The Health and Social Care Act 2008: ‘Code of Practice about the prevention and control of infections and related guidance. An appropriate risk assessment was in place, but the provider had not reviewed the recommended actions in the report. As a result, checks on water temperatures were not consistently carried out on the correct sentinel outlets, and lesser used taps were not flushed to prevent water stagnation. There were effective procedures to reduce the risk of Legionella, or other bacteria, developing in dental unit waterlines, and water quality testing was carried out.

The practice had protocols to ensure effective cleaning and safe segregation and disposal of hazardous waste.

Equipment was maintained and serviced in line with manufacturers’ instructions.

The practice completed infection prevention and control audits in line with current guidance.
