Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity by referring patients to these programmes as required. The practice was a veteran accredited practice, patients were prompted to register their veteran status, and this was advertised in the waiting room. The practice was proactive in ensuring patients with a learning disability received their annual review and scheduled this with their vaccination programme to minimise the number of visits for patients.

Monitoring and improving outcomes

Score: 3

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service performed better than the England average in relation to several cancer screening programmes, including screening for breast, bowel and cervical cancer. For example, the cervical screening targets of 80% were exceeded with the practice scoring 90% in both age groups measured. Breast screening coverage was 73% compared to the England average of 70% and bowel screening coverage was 81% compared to the England average of 72%.

Data from the UK Health Security Agency, March 2025, showed the 90% target for childhood immunisations was exceeded in 2 of the 5 areas measured. For the other 3 areas only 12 children were eligible, and the non-attendance of 2 children had had a significant negative impact on the data. The practice manger had evidence from other more recent data to show that they were up to date with childhood immunisations.

The practice completed regular audits to ensure care was provided for patients who required regular monitoring or were prescribed high risk medicines. The outcomes of the audits were discussed at clinical meetings and learning was shared.
