Assessing needs

The provider assessed people’s needs and choices. They met with people and their families to ask them about what they wanted from the agency. People told us they were consulted. Their comments included, “I was involved in writing the care plan” and “Everything is well organised, and we are asked for our views.” The provider developed care plans based on assessments. These were regularly reviewed and updated.

Delivering evidence-based care and treatment

The provider ensured people received evidence-based care and support. Staff completed a range of training including specialist training about learning disabilities and dementia. Managers met with staff to discuss best practice. A manager at the service was a qualified trainer. The management team were developing and delivering bespoke training. Staff told us this was helpful. Their comments included, “Training has helped build my confidence” and “The agency provided a good initial induction and ongoing training.”

How staff, teams and services work together

Staff worked well with each other sharing information. Staff told us they were informed about changes in people’s needs. Staff had opportunities to discuss their work with managers. The provider used messaging systems and electronic care recording to help ensure all staff had up to date and accurate information.

There were regular team meetings. Staff told us these were useful.

Supporting people to live healthier lives

The provider supported people to live healthy lives. They made sure care plans included information about people’s health conditions. Staff monitored these and liaised with families and other professionals when needed to discuss changes in people’s health.

Monitoring and improving outcomes

Staff monitored people’s outcomes and wellbeing. People using the service and relatives confirmed this. Their comments included, “They always check [person] and let me know any problems” and “The carers know all about [person’s] health and monitor [them] well.”

Staff kept records of the care they provided. Managers were able to view these in real time using the electronic care recording system. This meant they could address any concerns promptly.

The provider ensured people consented to their care and treatment. They discussed this with people. People told us staff offered choices and obtained consent when providing care.

Staff undertook training about the Mental Capacity Act 2005 and understood their responsibilities.
