Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

The services website was easy to navigate and provided patients with information on common conditions and support options. The website contained links to external websites such as the NHS’s better health page and links to useful self-help tools. The waiting area contained multiple information boards on different subjects, these were kept up to date and relevant.

The service held a register of carers who were all coded on the system. The service contacted these patients to ensure all information was kept updated. They held a bi-monthly carer’s cafe which provided a welcoming space for carers to connect, share experiences and explore ways of supporting each other. The service had 25 people on the learning disabilities register and had completed 91% of the annual reviews.

Between April 2025 and March 2026, the service saw 14% of eligible patients for NHS health checks. Information promoting health checks was clearly displayed to encourage engagement. The service audited their recall process and implemented changes, which included introducing dedicated clinics and sending out booking invites for patients to book their own appointments.

The service demonstrated an increase in uptake from 87 to 234 patients and was subsequently ranked 7th out of 89 practices across Leeds for performance in this area.

Care for patients with more complex needs was well coordinated. The service worked closely with external partners to support palliative care and had adopted an evidence-based improvement framework. Additional initiatives included weekly outreach clinics for patients affected by drug and alcohol misuse and recognition as a veteran accredited service. Staff were proactive in identifying health risks, particularly for vulnerable groups, and supported national priorities such as smoking cessation and reducing obesity.

The 2025 National GP Patient Survey showed that 77% (national average 69%) of patients felt they had enough support from local services or organisations to manage long-term conditions,

The service’s cervical screening uptake rate as of June 2024 was 83.2% for women aged 25-49 and 85.8 % for women aged 50-64. The service provided unverified data showing that the current cervical screening data was slightly above the figures we had available to us. The service took steps to encourage patients to attend cervical screening, for example, by sending regular recalls, offering extended access clinics and conducting opportunistic screening. The service was above the national averages for bowel cancer screening, with an uptake rate of 74.4% compared to the national rate of 63.5 %, and above the national average for breast cancer screening, the national average being 67.1% and the service uptake rate at 77.8%.

The service had met the 90% minimum requirement for the majority of immunisation indicators and had exceeded the World Health Organisation’s recommended uptake rate of 95% for one indicator. The service also provided unverified data indicating performance above 90% across most indicators, demonstrating strong overall immunisation uptake.

Evidence seen during the inspection indicated that the practice was the highest performing of 87 practices in Leeds for cervical screening uptake, influenza vaccination uptake and immunisations for children at one year of age.

We spoke to staff who could outline the systems the service had in place to recall and review patients, and they understood their role in this process.
