During an inspection looking at part of the service

This inspection complements the previous inspection undertaken on 7 June and 12 June 2013 and makes use of advice given to us from specialists within the Care Quality Commission and a specialist dental adviser, who accompanied us on the day of our inspection.

We found assessments of people's needs were not always thorough and confirmation as to why treatment was necessary was not always evident.

People who use the service were not always protected against the risks of unsafe or unsuitable premises because appropriate risk assessments had not been undertaken.

The provider did not have a safeguarding policy in place and staff had not undertaken appropriate safeguarding training.

People were not protected from the risk of infection because appropriate guidance had not been followed.

Staff were not always supported to deliver care and treatment safely and to an appropriate standard because training was not provided.

The provider did not have an effective system to regularly assess and monitor the quality of service that people receive.

During a routine inspection

People told us they could make informed choices about their dental treatment. One person told us, "He always shows me what he is doing, with a mirror, and explains it very thoroughly." Another person said, "He explained about the dentures, how they are made and what the problem was."

People we spoke with were complimentary about the care and treatment they received. Their comments included, "Excellent. Can't fault them at all" and "I went in and the service is absolutely fantastic. I felt welcome the moment I walked through the door." One person said, "I'm happy with everything. I'm delighted with the treatment." We found that people's needs were assessed but care and treatment was not always planned and delivered in an effective and safe manner.

Clinical staff working in the surgery had an up to date registration with the General Dental Council. However, some staff had not always received appropriate training and development.

The provider did not always have effective systems in place to monitor the quality and safety of the service.

Appropriate procedures were not in place, and staff did not have sufficient training and awareness, to deal with issues in relation to protecting people from the risk of abuse.

There were not always effective systems in place to reduce the risk and spread of infection.

The provider had not taken appropriate steps to provide care in an environment that was suitably designed.
