Effective
Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.
At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs
The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.
The provider carried out comprehensive initial assessments and risk assessments, and care plans were developed to manage those risks.
People and relatives told us the support provided met their needs and was reliable.
Delivering evidence-based care and treatment
The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.
The service used recognised tools to assess people’s needs. They also requested external help from relevant external specialists, when required. Staff told us how they monitored people’s nutrition and hydration where required, to manage specific risks to people’s care.
People and relatives told us staff had access to care plans that guided how care should be provided, and this was followed.
How staff, teams and services work together
The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.
Staff told us they worked well as a team and they had access to information that was up to date about people’s needs. Senior care workers completed regular visits to people and completed observations to ensure consistency of working in line with expected standards. The office team and registered manager held weekly operations meetings to review relevant aspects of the service delivery.
Staff told us there was strong internal communication, mutual support during pressures and accessible managers.
There were systems in place to ensure staff worked alongside other health professionals in an effective way.
Supporting people to live healthier lives
The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.
Staff encouraged people to regain or maintain skills, for example after a health incident, by promoting activity and safe community access.
Monitoring and improving outcomes
The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.
People’s care was regularly reviewed and changes put in place if required.
The service regularly sought feedback to people and relatives about the service being provided, to ensure good outcomes and people’s preferences continued to be met.
Consent to care and treatment
The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.
Staff told us how they sought consent at each visit.
Policies, training and supervision supported staff to apply the principles of the Mental Capacity Actin practice, and records showed regular contact with people and relatives to check consent and preferences were understood and followed.
