Supporting people to live healthier lives

Score: 3

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focused on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including NHS health checks and stopping smoking.

The service provided information to support and raise awareness among carers. This was available both on a noticeboard in the waiting area and on the service’s website. The service had a designated carers champion who maintained a register of people identified as carers and provided them with relevant support and information.

People could access a range of health promotion information in the waiting areas through TV screens, posters and leaflets designed to support healthier choices.

The service, with the help from the PPG, organised a self-care event at the service during the national self-care week in November 2024 to promote healthy living by delivering health messages and advice.

The service was supported by the health and wellbeing team from the primary care network (PCN) which provided support with housing, finance and signposted people to relevant services. (A PCN is a group of general practices that work together to provide a wider range of services to the local population.)

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service performed better than the England average in relation to several cancer screening programmes, including screening for breast and bowel cancer.

National data (2024-2025) showed the service had achieved the 90% minimum uptake for all 5 indicators for childhood immunisations, and 4 of them reached the 95% target recommended by the World Health Organization (WHO).

Latest national data for cervical screening (2024) showed the service had met the national update target of 80% for people aged 50-64 years old but had not met the same target for people aged 25-49 years old. The service had taken actions to improve this. For example, the service made the appointments more accessible by offering appointments on Saturdays. The service called eligible people who did not respond to the invitation for screening to explore the underlying reasons and addressed their concerns, aiming to improve the screening uptake. Latest unverified data from the service showed increased cervical screening uptake.

The service had systems and processes to offer people with a long-term condition an annual review to monitor people’s care.
