How staff, teams and services work together

Score: 3

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

Staff told us they used a handover to share information about changes in people’s needs and health with each other from one shift to another.

Feedback from health professionals was positive about how they worked with the service. One health professional said, “The staff are very approachable and are always willing to accept advice and recommendations.” Another professional told us they felt paper-based communication systems could improve, but felt staff were good at taking and acting on verbal handovers from their team.

Leaders felt they had a good relationship with most health professionals. However, leaders also told us they had identified a breakdown in communication with one health partner. At the time of the assessment, they were considering what action to take to improve communication and strengthen the working relationship.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

The service had effective processes in place to ensure people’s health and wellbeing needs were met. For example,

One relative said, “Since [Person’s name] have been there [Person’s name] has put weight back on and they (Staff) let me know when [Person’s name] needs more treats. I am really pleased now [Person’s name] has a quality of life now that they had lost.”

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

Staff confirmed how they monitored and recorded people’s health and the actions they took if they were concerned about changes in people’s health needs.

The registered manger confirmed they had oversight of people’s records and said they spent a lot of time with people. They said they would also pick up if someone’s health needs had changed and take appropriate action.

Leaders said the service had the same GP for continuity and they come into the service to see people weekly.
