We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Staff regularly reviewed people’s care and worked with other services to achieve this. At our last assessment, we rated this key question as good. At this assessment, the rating remains the same.

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. The service had effective arrangements in place to provide a range of health checks, including NHS Health Checks for eligible people, physical health checks for people with a severe mental illness, and annual health checks for people with a learning disability. In the past 12 months, all eligible people with a learning disability were offered a health check, and the practice had met their target to help ensure people remained healthy. Additionally, the practice had exceeded their set target for NHS annual health checks in the last 12 months, which helped people to live healthier lives.

Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity. A range of information was available in the waiting room which signposted people to information and local and national support groups to encourage healthier living.

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.

The service met national targets for the uptake of childhood immunisations except in 1 area which was slightly below (87%) the national average of 90%. The practice told us they had taken significant action to address this shortfall and documents reviewed confirmed this. Cervical screening data showed uptake was slightly below the 80% target in the 25 to 49 age group at 79% and in the 50–64 age group at 76%. Staff told us this had increased, and the practice provided unverified live data which showed us the target had been achieved for both age groups at 82% and 83% respectively. Staff told us the practice had worked to improve attendance by ensuring they had effective recall systems, and flexible appointment times, which included weekly late evening and Saturday appointments. Screening for breast, and bowel cancer was slightly better than England average.
