Assessment processes were in place to ensure people’s needs were identified and planned for. People were involved in the process and felt their views and opinions were listened to and incorporated into their care plan. People’s care plans were updated in line with any changes in their needs to ensure it recorded accurate information about how people wished to be supported. Some care records lacked detail about people’s cultures and how they were to be supported to ensure a holistic approach to care delivery. The registered manager told us this information was known and they would ensure it was captured in people’s care records.

Assessing needs

Score: 3

Staff identified and supported people with their needs. They told us they met with and discussed with people the level of support they required and used that information to identify care and support needs. A staff member told us, “When a person first starts, we go through what their care needs are. The first call is about learning about what they want and what they need.” The registered manager said, “On our assessment we try to find out more, to find out about person centred care from the client’s point of view.” They told us they continued to assess people’s needs and reviewed care delivery to ensure it remained in line with people’s needs. They said, “We update our care plans as and when the need arises and as their condition changes. The care plan is a living document. It’s an active thing that is dynamic and changes with the person.”

People’s needs were assessed and planned for. Processes were in place to gather information, in discussion with people, about their needs and how they wished to be supported. Care records identified people’s physical support needs and how they wished to be cared for. There was some information in people’s care records about their life histories and religious preferences, but records did not contain information about all of people’s preferences and how they wished to be supported. For example, in relation to cultural preferences and preference of gender of the care worker supporting them. The registered manager told us they would ensure that care workers knowledge about the people they were supporting was recorded in their care records.

People felt involved in the assessment process and the development of their care and support plan. One person told us, “Before I started having the care workers, someone came from the service and talked to me about what help I might need and between us we decided what would be best for me and what the care workers would do for me.” A relative confirmed this process as well. They said, “The manager came from the office and went through everything mum might need and we agreed together what the care workers would do for her when they came.” People and relatives confirmed that care plans were in place which detailed the conversations they had with the manager and were updated as people’s needs changed. One person said, “My care plan was done with me and they wrote everything I asked them to.”
