Person-centred Care
The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.
Staff demonstrated a strong understanding of people as individuals, including their interests, life histories and what brought them comfort and meaning. Care plans reflected people’s wishes and how they wanted to be supported when decisions were made. For example, one care plan clearly described the person’s wishes should they approach the end of their life, including specific music they would like to be played. Staff understood people’s preferences and worked with them, their relatives and other professionals to respond to changes in their needs. Care plans were regularly reviewed and updated to ensure support remained appropriate and responsive.
A staff member said, “If someone has communication difficulties, we adapt the way we communicate so they can understand and feel included. Another staff member told us, “Residents are treated with dignity, and teamwork is one of the home's strengths.”
