Our view of the service

Date of Assessment: 01 May and 27 May 2026. Middleton Park Lodge is a nursing home, registered to provide personal and nursing care to 43people. At the time of the assessment, there were 42 people living at the home. We completed this assessment following the previous rating of the service.

The provider demonstrated some strengths in maintaining a safety culture, delivering effective care, and promoting person-centred approaches, supported by assessments, care planning, and partnership working. However, these were undermined by inconsistent practice, gaps in staffing, and weaknesses in governance. Systems to monitor risk, quality, and improvement were not always effective, meaning concerns were not consistently identified or addressed. This included an incident where a person’s dignity was not appropriately maintained. Issues such as environmental risks, infection control, inconsistent documentation, and limited staff confidence in leadership impacted the provider’s ability to deliver consistently safe, high-quality care within an open and transparent culture.

The provider had begun to implement changes, including the introduction of new leadership and the development of plans aimed at driving improvement. While these actions demonstrate a commitment to addressing the concerns identified, it is too early to determine the overall impact on outcomes for people.

At our previous assessment of the service, the provider was found to be in breach of regulations relating to safe care and treatment and good governance. At this assessment we found enough improvements had been made in relation to safe, care and treatment and the provider was no longer in breach of this regulation. However, not enough improvement had been made in relation to good governance and the provider remained in breach. We also found concerns relating to how peoples dignity and respect was maintained and found the provider to be in breach of this regulation.

People's experience of this service

People’s experiences were variable. While most received personalised care and were supported to maintain their wellbeing, there were inconsistencies in how care was delivered day to day. Not all people were treated with dignity, respect, and responsiveness at all times, and opportunities for engagement were not always inclusive. Although relatives often provided positive feedback, “they are so compassionate,” and people were involved in decisions about their care, gaps in staffing, inconsistent practice, and variable leadership support affected the delivery and consistency of compassionate, person-centred care.
