Assessing needs

Score: 3

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

People’s needs were thoroughly assessed prior to them moving to the service. The manager told us, “We get some details first from themselves, their family member, or [the local authority]. We then book a face-to-face assessment, talk through all our assessment process. It takes 1-2 hours to go through everything.”

We did not receive any concerns in relation to the assessment process during our inspection.

Delivering evidence-based care and treatment

Score: 3

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. Evidence based tools were used to assess people’s risks. For example, tools to assess the risk of pressure areas and malnutrition. People’s care and support plans were regularly reviewed to ensure they remained current and up to date. People told us staff supported them in line with their care plans.

How staff, teams and services work together

Score: 3

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

Staff worked well together as a team. Staff told us there were regular team meetings and we saw evidence of these. There were other processes in place to support effective team working such as a messaging platform and staff get togethers.

Supporting people to live healthier lives

Score: 3

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

There was detailed information in people’s care plans about their health conditions and how staff could support with these. People had information in their care plans about other health professionals they worked with for continuity of care.

Monitoring and improving outcomes

Score: 3

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

People’s outcomes were recorded within their care plans for each aspect of their care. People’s care plans were reviewed regularly to ensure these outcomes were monitored. The manager regularly sought feedback from people to enable them to improve outcomes, for example, socially.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

People’s capacity was assessed where required and appropriate best interest decisions were in place. Staff had received training in the Mental Capacity Act (MCA). People said staff sought their consent before supporting them.
