During an inspection looking at part of the service

We undertook an announced focused inspection of Upper Denby Dental Care on 11 November 2021. This inspection was carried out to review in detail the actions taken by the registered provider to improve the quality of care and to confirm that the practice was now meeting legal requirements.

The inspection was led by a CQC inspector who was advised by a specialist dental adviser.

We undertook a comprehensive inspection of Upper Denby Dental Care on 18 August 2021 under Section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We found the registered provider was not providing well led care and was in breach of Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. When one or more of the five questions are not met, we require the service to make improvements and send us an action plan. We then inspect again after a reasonable interval, focusing on the areas where improvement was required.

The provider had made improvements in relation to the regulatory breaches we found at our inspection on 18 August 2021.

Background

Upper Denby Dental Care is on the outskirts of Huddersfield and provides private dental care treatment for adults and children.

There is disabled access at this location with treatment rooms based on the ground floor. Car parking spaces are available at the practice car park.

The dental team includes two dentists, one dental hygienist, a practice manager and dental nurse/receptionists. The practice has two treatment rooms.

The practice is owned by a company and as a condition of registration must have a person registered with the CQC as the registered manager. Registered managers have legal responsibility for meeting the requirements in the Health and Social Care Act 2008 and associated regulations about how the practice is run. The registered manager at Upper Denby Dental Care is the principal dentist.

During the inspection we spoke with the principal dentists and the practice manager. We looked at some staff records, risk assessments and other records about how the service is managed.

The practice is open:

Monday to Friday 8:30am-5pm.

Our key findings were:

  • The provider’s recruitment of staff followed the organisations own recruitment procedure and reflected current guidelines.
  • Improvement had been made to the oversight of staff training and staff had completed sepsis awareness training.
  • Improvements had been made to the electrical safety in the building with a fixed wiring examination and some additional safety measures being put in place.
  • A legionella risk assessment was in place and records showed the monitoring of water temperatures. Key staff had booked training in legionella awareness and management.
  • Patients dental care records were now regularly audited to ensure all detail required by General Dental Council standards were included.

During an inspection looking at part of the service

We carried out this announced focussed inspection on 18 August 2021 under section 60 of the Health and Social Care Act 2008 as part of our regulatory functions. We planned the inspection to check whether the registered provider was meeting the legal requirements in the Health and Social Care Act 2008 and associated regulations. The inspection was led by a Care Quality Commission, (CQC), inspector who was supported by a specialist dental adviser.

To get to the heart of patients’ experiences of care and treatment, we asked the following three questions:

• Is it safe?

• Is it effective?

• Is it well-led?

These questions form the framework for the areas we look at during the inspection.

Our findings were:

We found this practice was providing safe care in accordance with the relevant regulations.

Are services effective?

We found this practice was providing effective care in accordance with the relevant regulations.

Are services well-led?

We found this practice was not providing well-led care in accordance with the relevant regulations.

The practice is open:

Monday to Friday 8:30am-5pm.

Our key findings were:

  • The practice appeared to be visibly clean and well-maintained.
  • The provider had infection control procedures which reflected published guidance.
  • The provider had implemented standard operating procedures in line with national guidance on COVID-19.
  • Staff knew how to deal with emergencies. Appropriate medicines and life-saving equipment were available.
  • The provider had systems to help them manage risk to patients and staff.
  • A legionella risk assessment was in place, we found records did not show correct monitoring of water temperatures and key staff had not completed training in legionella awareness and management.
  • The provider had safeguarding processes and staff knew their responsibilities for safeguarding vulnerable adults and children.
  • Patients dental care records did not include all detail required by General Dental Council standards.
  • Staff provided preventive care and supported patients to ensure better oral health.
  • Staff were involved, supported and worked as a team.
  • The provider’s recruitment of staff did not follow the organisations own recruitment procedure or reflect current legislation and the provider did not have oversight of staff training.

We identified regulations the provider was not complying with. They must:

  • Establish effective systems and processes to ensure good governance in accordance with the fundamental standards of care

9 September 2013

During an inspection looking at part of the service

We carried out this visit to check if changes had ben made to the way in which consent was recorded and, the way in which infection prevention control procedures were followed.

At this inspection, we found the provider had introduced new consent paperwork which involved the person signing a consent to treatment form for every course of new treatment. We looked at five patient records and saw they had all signed a consent to treatment form.

We looked at the new infection control paperwork which had been introduced. We saw that a log was kept of the daily and weekly checks for the ultrasonic bath and steriliser. We saw the practice had audited themself against the government decontamination guidance, HTM105, and there were no actions for them to address. We also saw that a Legionella risk assessment had been conducted in August 2013. The risk assessment showed that the risk of legionella developing was low.

During a routine inspection

During our inspection, we were able to speak with two people who use the service. They told us they were very satisfied with the quality of services provided; treatments including costs were fully explained in order for them to make informed choices.

People told us they were happy with the staff at the practice. People's comments about staff included:

"The staff are superb"

"The staff are great and I like the practice very much."

The environment of the waiting area and the surgeries of the dental practice were visibly clean and tidy.

During our visit we spoke with two of the Dental Nurses/ Receptionists who told us that they felt they were supported to be able to undertake their job effectively and enjoyed working at the practice.
