Medicines optimisation

People were given their medicines in a safe way, and people we spoke with were happy with the way they received them.

When medicines were given, they were recorded on Medicines Administration Record charts (MARs). These showed that people generally received their medicines in the way prescribed for them.

When medicines were prescribed to be taken ‘when required’ there was information in place to guide staff when these might be needed. These had not always been updated after changes to people’s medicines. However staff were knowledgeable about people’s needs and requirements.

When medicines were given covertly then people’s mental capacity was assessed, best interest decisions taken, and the situation discussed with prescribers. However covert administration forms had not always been fully completed or reviewed in line with the provider’s medication policy.

Information about people’s medicines was recorded in their care plans. One person’s care plan gave conflicting information about when a dose of their medicines should be given in relation to food.

There were suitable arrangements for medicines needing cold storage and those requiring extra security. Suitable temperature monitoring was carried out to make sure medicines would be safe and effective. Medicines were stored securely, however some medicines waiting for disposal were stored in an area accessible by some other staff not authorised to handle medicines. Since our visit the provider has confirmed that systems have been changed to ensure there is no unaccompanied access to medicines going forwards.

Staff had training and competency checks to make sure they gave medicines safely. We were told that some annual staff checks needed to be updated. There were systems to report and investigate any incidents to try to reduce the chance of recurrence.
