We found this practice was not providing effective care in accordance with the relevant regulations. We will be following up on our concerns to ensure the provider has made the required improvements.

During our inspection of this key question, we found concerns related to:

the completeness of patient care records

practice’s procedures for obtaining patient consent to care and treatment did not comply with recognised guidance.

the practice not having an effective system to monitor patients’ referrals to other dental or health care professionals.

These concerns were in breach of Regulation 17, Good governance.

The provider had systems to ensure that people who used the service received person-centred care and treatment, that was appropriate, met their needs and was reflective of their personal preferences. Preventive oral health care was encouraged.

We looked at 6 patient care records. The information recorded in these patient care records was mostly in line with recognised guidance. However, risk assessments were not documented and

radiographs were not routinely justified or graded.

Staff told us they obtained patients’ consent for treatment in line with legislation and guidance and understood their responsibilities under the Mental Capacity Act 2005. However, we did not see evidence that treatment plans and associated costs were provided to patients.

The practice had systems for prescribing of medicines in line with current guidance. Prescription pads were kept securely, however, there was no log in place to monitor and track their use. We reviewed one antimicrobial prescribing audit that did not contain sufficient information to effectively assess prescribing practices. There were no previous antimicrobial prescribing audits available for review.

Patients were referred to primary and secondary care specialists for treatment the practice did not provide. This included referring patients with suspected cancer under the national faster diagnosis pathways. Improvementswererequiredto ensure referrals to other healthcare professionalswere centrallymonitoredto ensure they were receivedin a timely mannerand not lost.
