Person-centred Care

Score: 3

The provider made sure people were at the centre of their care and treatment choices and they decided, in partnership with people, how to respond to any relevant changes in people’s needs.

People were at the centre of their care and support. Support being delivered was in line with people’s wishes and needs. Requests made by people were followed and actions taken to facilitate them. For example, when they chose what they wanted to do that day and where they wanted to go.

Independence was promoted for all people. Those who were safe and able to managed their own medicine, or with minimal support. Communication systems were individual to each person and clearly outlined in their care plan. All staff were aware of the communication methods people used and we saw this in practice.

Activities were person-centred. Effort was put in by the management and staff to ensure these were bespoke. For example, 1 person loved cars and so a regular activity was set up with a local garage.

Now care plans were being developed with people to make them as interactive as possible. The management had identified this as an area of improvement. An electronic care plan system was having all the information inputted into it and care plans were updated ready for this. They were exploring with the company ways to support people inputting information into their own care plans.

When changes were required, these were made. Although occasionally the paperwork had not been updated in a timely way. The management had already identified person-centred care plans as an area for development.
