We saw that in the time since taking ownership of the practice, the provider had not always demonstrated sufficient oversight which had led to lapses in the governance arrangements. We noted that a number of mandatory training activities, equipment servicing and risk assessments were undertaken immediately prior to the inspection.

We saw that processes for learning, quality assurance and continuous improvement were not being completed according to recognised guidance. The provider should take action to ensure audits of radiography, infection prevention and control, record keeping and antimicrobial prescribing are undertaken at regular intervals to improve the quality of the service. The practice should also ensure that, where appropriate, audits have documented learning points and the resulting improvements can be demonstrated.

We were told that a new leadership team had been put in post within the last month. We were shown the newly implemented governance systems to prevent such occurrences from being repeated. We were told that oversight arrangements would be strengthened, policies and procedures reviewed at appropriate intervals, in order to achieve ongoing compliance.

Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations.

There were effective processes for receiving and acting on safety alerts.

Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement.

Staff feedback was obtained through meetings and informal discussions. They were encouraged to offer suggestions for improvements to the service, and said leaders were approachable and open.
