Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice.

The provider completed in depth analysis and trends of accidents and incidents. Any incidents were mapped out visually to identify any risks or hazards to people’s environment. The registered manager discussed incidents with staff to monitor and learn from. This ensured a consistent and person-centred approach to supporting people.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services.

Health professionals visited the service regularly to support people and complete assessments and reviews. Feedback from a visiting professional was provided to reflect how well staff support people.

Admission processes were in place to ensure people’s needs were fully assessed before offer of a placement. Health passports were in place for when people had to attend healthcare settings in the community. The passports reflected people’s health and care needs, how they communicated and received care and any long-term conditions.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. People told us they felt safe living at Manna Walk and this was echoed by relatives. One relative said, “It's a good safe place and I have never had any concerns over safeguarding.”

The provider shared concerns openly and in a timely way with external stakeholders such as the local authority. Staff were trained to identify and manage any situations where people’s safety or human rights may be compromised. Where people’s liberty had been restricted, the provider had ensured this was done lawfully using the Deprivation of Liberty Safeguards process.

Involving people to manage risks

The provider worked with people to understand and manage risks. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

Positive interactions were observed, staff knew people well and their needs. Staff communicated effectively with people and understood people’s communication preferences. Daily records were completed consistently and highlighted any concerns or actions should there be a change in staffing. Staff were proactive in managing behaviours that may challenge and were responsive to people's needs, whilst also ensuring people’s safety.

There was a strong leadership team who worked together cohesively to manage risk. People were involved in their support and risk management plans. Plans in place were person centred; outcome focused with structured goals and aspirations recorded.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. Systems were in place to monitor and regularly check the safety and maintenance of all areas of the premises and equipment used in the delivery of care. The maintenance team carried out necessary repairs and undertook regular checks and audits to monitor safety.

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs.

We observed there were enough staff deployed throughout the service to meet people’s needs when required. A relative told us, “Staff are friendly and approachable. They are transparent and clear and very much understand [persons] needs.”

Staff had access to a range of training courses and NVQ qualifications. The registered manager completed competency checks to monitor staff knowledge and practice. Staff told us they were happy with the training.

Staff were recruited safely and completed induction and essential training related to people’s health, safety and welfare. Management completed supervisions and staff meetings to support the staff team and help make sure training was effective.

 

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly.

Staff received training on infection control. There was liquid soap and paper towels and hand washing areas and additional hand sanitisers throughout the service. There was a good supply of personal protective equipment (PPE). Staff were trained in PPE use.

Medicines optimisation

The provider made sure medicines and treatments were safe and met people’s needs, capacities and preferences. Staff involved people in planning, including when changes happened.

People’s medicines were administered safely. Medicines were stored securely following current guidelines for the storage of medicines.Staff had received training, and competency checks were completed to ensure staff were suitably skilled to do their roles.

The medicines administration records contained the necessary information for safe administration of people's medicines.
