Governance, management and sustainability
The practice had a governance system that included policies and procedures, which were accessible to staff. Improvements were required in regard to policies being reviewed on a regular basis. For example, the complaints policy had some outdated contact details, and the Disability Access Audit mentioned resources the practice did not have. We spoke to the provider about this and they told us they would review their policy documents.
Systems and processes were embedded, and the inspection did not highlight any significant issues or omissions. Areas requiring improvement were acted on immediately.
Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations.
There were some relevant policies and protocols were in place for the use of closed-circuit television (CCTV).However, the CCTV signage did not give details of who was carrying out the surveillance. We spoke to the provider about this and they told us they would update the signage.
There were some processes for identifying and managing risks, investigating incidents and accidents, and for receiving and acting on safety alerts. Improvements were required in regards to health and safety risk assessments. We found that the risk assessment had not identified a number of hazards including staff room door hinge coming off, and the carpet lifting in the hallway leading to the staff room. We spoke to the provider about this, and they told us they would address these issues and ensure risk assessments were reflective of the current conditions within the practice going forward.
The practice had systems and processes for learning, quality assurance and continuous improvement. This included undertaking audits that were carried out 6-monthly according to recognised guidance.
Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and for improvement.
Staff had clear responsibilities, and systems of accountability to support good governance.
Staff feedback was obtained through meetings and informal discussions. They were encouraged to offer suggestions for improvements to the service, and they said these were listened to and acted upon, where appropriate.
