Our view of the service

We inspected this service on the 7 May 2026 as part of our ongoing schedule of assessments.

Gladstones Cotswolds Clinic provides accommodation and treatment for up to 12 clients who require residential substance misuse treatment which can include medically monitored detoxification from alcohol and opiates. The service only accepts privately funded clients. At the time of this inspection there were 10 clients at the service. The service had a registered manager in post.

At the last inspection we found 3 breaches of regulation. At this inspection we found that there had been improvements in 2 of those areas. Staff were now receiving supervision and clients risk assessments were being updated following incidents. At the past inspection we also found that there was limited evidence of a structured governance system. At this inspection we saw that there had been some improvement to the governance system. However, management staff were still not tracking clinical progress or measuring outcomes as part of their governance system. This made it hard to discern how the service was performing, which meant that they could not easily target areas to improve.

At this inspection we found 3 breaches of regulation.

  1. We found that there was a breach in the governance of the service.
  2. A breach in their statutory notification duties to the Care Quality Commission (CQC) and local safeguarding authorities.

3. There was a breach in regulation surrounding blanket restrictions at the service.

We have asked the provider for an action plan in response to the concerns found at this assessment.
