We looked for evidence staff involved people in decisions about their care and treatment and provided them with advice and support. Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support.

Staff focussed on identifying risks to people’s health, including those in the last 12 months of their lives and those with caring responsibilities. Staff supported national priorities and initiatives to improve population health, including stopping smoking and tackling obesity.

To support wider health and wellbeing, the practice worked alongside a health coordinator provided the local primary care network. The coordinator proactively contacted eligible patients to offer referrals into local smoking cessation and obesity management schemes.
 

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured outcomes were positive, consistent and met the expectations of people themselves.
The data for 2023/2024 was showing that the practice had fallen slightly below targets for childhood immunisations and cervical screening. However, during the inspection we looked at unverified current data and the practice was able to demonstrate that they had achieved achieving targets.

The practice offered appointments during extended hours services, which helped patients who could not attend during normal working hours, which improved uptake of screening and childhood immunisations. Additionally, staff regularly checked who was due or overdue for screening and sent reminders. As a result, more patients were able to access screening at a time that suited them.
