Our view of the service
Date of assessment: 30 June 2026 to 7 July 2026. Choices for Living Well is a short term reablement service, offering support to people in their own homes. They are registered to provide personal care.
At the time of our assessment, there were 43 people in receipt of care at the time of our first site visit. Due to this being a short-term service the number of people supported fluctuates daily. As the service had not yet been rated, we looked at all quality statements and domains to provide an initial rating.
The registered manager was absent from the service at the time of our inspection; therefore, a temporary service manager had been appointed.
Feedback was received from 5 health and social care professionals, who highlighted the good working relationships the provider had with partner agencies.
Improvements were needed in relation to care records. Whilst records provided basic information, people's wishes and preferences were vague and required further detail to guide staff in meeting people's specific needs, such as catheter care and the management of prescribed medicines.
Staff understood their roles and responsibilities and demonstrated a good understanding of people's individual needs. Staff stated there was a strong sense of teamwork; however, communication from management regarding the service changes could have been strengthened.
The provider had identified a need for improvements in relation to revising staff roles and the governance and oversight management had. A transformation plan has been created and managers met regularly to review the progress they had made.
People's experience of this service
Overall, people thought the service was well organised and managed, and team leaders and co-ordinators were professional and experienced. One person told us, “They work very hard and are organised well.”
People said they were supported by a wider team, including physiotherapists and occupational therapists (OT). Other professionals were contacted as and when appropriate. One relative told us, “They have referred to the physio and OT. They work well together to provide appropriate equipment”.
Staff listened to what people had to say. One person said staff are, “Fantastic. Very knowledgeable and so caring and considerate. The care I have had is exceptional.”
All professionals we spoke with, reflected on the positive working relationships they had with the provider and how knowledgeable staff were in relation to people's needs, despite having worked with them for only a short period of time.
All people we spoke with stated they would recommend the service.
